Companion Care

Companion Care provides assistance with errands, organization, as well as socialization. This type of service helps fight against isolation in the elderly and encourages socialization and activities while at home.

  • Transportation of client

  • Medication support

  • Light housekeeping: laundry, making bed, vacuuming, organizing, etc.

Senior Assistance

 Senior Assistance provides a helping hand in all activities of daily living. This includes all aspects of companion care with the addition of hands-on-care. 

  • Errands: Grocery shopping, picking up medications, doctors appointment, etc 

  • Transportation of client

  • Medication support

  • Light housekeeping: laundry, making bed, vacuuming, organizing, etc.  

  • Meal planning/Assisting with feeding

  • Toileting/bathing/ incontinence care

  • Dressing

  • Hygiene care

  • Assisting with transferring

  • Basic vital check

What is Dementia Care?

Dementia, or Memory Care, is an additional component that can be added to both Companion Care and Senior Care which focuses on:

  • Reminiscing/Life story activities

  • Maintaining a low stress environment

  • Activities to maintain mental and cognitive function

  • How to de-escalate and maintain communication

  • Practicing lifelong exercises that maintain strength and mobility

  • Maintaining a low stress environment

Long Term

Long Term services are those that last more than 3 months. Sable Home Care offers Long Term services to Companion Care and Senior Assistance.

Respite Care/Short Term

Respite Care/Short Term services are for those that only need care for a set period of time (three months or less). This would be a great option for those healing, transitioning from rehab, waiting for a place at an Independent or Assisted Living, or the main caregiver is on vacation!

Hospice Assistance

Hospice Assistance is for those that are signed on to Hospice Care and need extra assistance with Activities of Daily Living. There is no set time for these services. The focus on on comfort and bereavement.

Nursing Services

  • Medication Refilling

    Bi-monthly, a dedicated healthcare professional will visit to replenish your medication dispenser, ensuring seamless adherence to your prescribed regimen. Timely notifications will be provided to you or your designated family members, alerting them when medication supplies are low or nearing depletion. This proactive approach aims to minimize the risk of missed doses and optimize your medication management experience.

    Q: Is it necessary for either myself or my family member to be a client in order to access these services?

    A: No! These services are available to anyone residing in Grand Rapids or the surrounding areas within our designated territory.

    Q: Is the assistance limited to seniors or individuals with diabetes?

    A: No(: Our nursing services are open to individuals of all ages and health conditions. Anyone in need of these services is welcome to apply.

  • Foot Care/Nail Care

    Every six weeks, a qualified healthcare professional will provide a comprehensive foot care service, including a soothing foot soak, precise toenail clipping, and vigilant monitoring for any emerging changes or concerns. Recognizing the challenges associated with reaching and maintaining toenail health, our aim is to alleviate any additional stress while diligently overseeing the well-being of your feet, a critical consideration, particularly for individuals managing diabetes.
